Details about this service: This service is only for Blue Sky Bio Fully Guide Kit and Implant which includes planning a clinical treatment plan of One Implant Site which includes: 

importing CT scan and confirming the panoramic curve, drawing nerves, placing implants, abutments and virtual teeth. Includes designing of one implant surgical guide site using Blue Sky Plan software. 

You will be contacted to approve the digital treatment plan prior to the fabrication of the surgical guide. 

There will be a $21 charge to export the surgical guide from the Blue Sky Plan software, printing the surgical guide (using an FDA compliant material) and inserting the relevant metal cylinders. 

There is a limit of one implant site for this service. 

If you request expedited shipping, you will be charged the following shipping rates. USPS $9, 2nd Day Air $23, Overnight $55.
