To be eligible:
· The case must be planned and executed with a Blue Sky Bio dental implant.
· The case must have parallelism in between adjacent teeth.
· The case must be scanned with a BSB scan body (scannable impression coping).
· The customer must purchase a Blue Sky Bio SKY-Base abutment for the case.
· The coupon code is for single implants site only.
· Cases of excessive implant depth or angulation are not eligible. Once planned and approved by the customer, the completed crown will be delivered from Innometive. If you request expedited shipping, you will be charged the following shipping rates.
